Is therapy HSA eligible?
Yes, when it treats a condition. Therapy for a diagnosed mental illness counts; marriage counseling doesn’t.
The rule
The IRS counts therapy that treats a disease. Its own example: therapy to treat a diagnosed mental illness is a medical expense, and marital counseling is not.
Psychiatrists, psychologists and psychoanalysis for medical care all count, and so do programs that treat a drug or alcohol use disorder.
Paid yourself, it waits in the HSA until you take the money out, this year or decades from now.
- What to keep
- The therapist’s receipts or statements, with dates and what you paid.
- Watch for
- Psychoanalysis taken as part of training to become a psychoanalyst doesn’t count.
- Anything insurance paid, or paid you back for, can’t also come out of the HSA. Only what you paid yourself counts.

Keep the receipt. Take the money out later.
Pay a medical bill yourself and Reimburse keeps the receipt, with who it was for, until you take the money out of your HSA tax-free.
