Is quitting smoking HSA eligible?

Yes. A program to stop smoking counts, and since 2020 an HSA can also pay for nicotine gum and patches, which the itemized deduction can’t count.

The rule

The IRS counts a smoking-cessation program because it treats a disease, tobacco use disorder.

Nicotine gum and patches are drugs sold without a prescription. Since the CARES Act, an HSA can pay for those for anything paid after December 31, 2019, though they still can’t go on Schedule A. A drug to quit that a doctor prescribes counts as a prescription.

Paid yourself, it waits in the HSA until you take the money out, this year or decades from now.
