Is cosmetic surgery HSA eligible?

Usually not. Surgery to improve appearance doesn’t count, unless it corrects a deformity from a congenital abnormality, an accident or a disfiguring disease.

The rule

A procedure aimed at improving appearance that doesn’t meaningfully promote the body’s proper function, or prevent or treat illness, isn’t medical care. The IRS names face lifts, hair transplants, hair removal and liposuction.

Surgery that improves a deformity from a congenital abnormality, an injury from an accident or trauma, or a disfiguring disease does count. Breast reconstruction after a mastectomy for cancer is the IRS’s example.

What to keep
For the exception, the surgeon’s records of what the surgery corrected.
